Listen, I'm not any sort of doctor and have no plans to become one; however, depression runs in my family, and what you're describing sounds like manic depression. Manic = your happy moods, depression = sad. Thus no, it has nothing (or little) to do with your eating/sleeping/exercise habits; it sounds like something chemical. (Which, yes, can come and go, so it's not necessarily odd that you just had a couple of 'good' months.)

Anyway, exercise is good for mild to moderate depression, sure - all those endorphins - but MANIC depression is a whole other animal, and I would suggest you get yourself checked out. Especially after hearing about the preoccupation with death. The thought of dying is not pleasant, agreed, but it should not freak you out to that extent. Again, my only degree of any sort is an almost-bachelor's in linguistics (and I won't even hold that till May), but as someone who's relatively well-informed on the various forms of depression, this post really concerns me. My advice: print all this out and take it to your doctor.

And by the way: I TOTALLY agree about the athletic clothes comment. ;)
